Outreach and Support

We believe that the sooner clients are back in charge of their lives and responsible for their own actions, the better off they will be. To help them achieve this, we supplement our programs with a solid case management methodology that begins with a client's first step through the door into our EntryPoint drop-in program or their first phone call. It ends several months after they have reached the fullest level of independence possible.

We also supplement all of our programs with our Health Services, since illness or injury can easily derail any progress someone has made towards self-sufficiency and is often at the root of homelessness.


EntryPoint Services

The EntryPoint Drop-In Program is operated out of our Chicago Ave. facility and is open for client drop-in on Monday, Wednesday, and Thursday mornings from 8:00 a.m. to noon. Clients have access to the following at no cost:

  • Clothing and toiletries
  • Laundry and shower facilities
  • Telephones and computers
  • Locker storage
  • Physical and mental health care screenings and monitoring
  • Clinical and recreational workshops

All clients go through an initial intake process that leads, for those who are ready, to regular case management meetings and development of goals that will move the client toward stability.


Health Services

Connections has a part-time nurse on staff and a team of volunteer doctors who provide routine screenings, checkups, and basic medical care, as well as referrals to other medical professionals as needed. We receive funding for and report on the following types of healthcare:

  • TB testing
  • Flu shots
  • Screenings, counseling and care for diabetes and heart disease
  • HIV/AIDS testing and counseling
  • Access to financial assistance in paying for medications
  • Medication management
  • Psychiatric and mental health screenings, referrals, and counseling
  • Substance abuse screenings, referrals, and counseling
  • First aid assistance
